Dentist On Kingsway collects, uses, and discloses personal health information in the course of providing dental care. This notice explains our practices regarding your personal health information under the Personal Information Protection and Electronic Documents Act (PIPEDA) and British Columbia’s Personal Information Protection Act (PIPA).
What Is Personal Health Information?
Personal health information includes information about your oral and general health history, examinations, diagnoses, radiographs and images, treatments, prescriptions, referrals, and related billing, insurance, and payment information.
How We Collect Your Information
We collect personal health information directly from you (for example, through medical/dental history forms and conversations with our team), and, with your consent or as permitted by law, from other sources such as referring or treating health professionals, dental laboratories, and your insurers.
How We Use and Disclose Your Information
- Treatment: to provide, coordinate, and manage your dental care, including sharing information with dentists, specialists, laboratories, and other health professionals involved in your care;
- Payment: to bill and collect payment, including submitting claims to your dental plan or insurer and verifying coverage;
- Practice operations: for appointment scheduling and reminders, recalls, quality improvement, staff training, and general administration;
- As required or permitted by law: including responses to court orders, regulatory requirements of the College of Dental Surgeons / applicable regulatory college, public-health obligations, or to reduce a significant risk of serious harm.
We do not sell your personal health information, and we will not use or disclose it for purposes beyond those described here without your consent.
Your Consent
We collect, use, and disclose your personal health information with your knowledge and consent, except where the law permits or requires otherwise. You may withdraw your consent at any time, subject to legal or professional restrictions and reasonable notice; withdrawing consent may affect our ability to provide care.
Safeguards
We protect your personal health information with administrative, technical, and physical safeguards appropriate to its sensitivity, including restricted access, secure storage, and staff privacy training.
Retention
We retain dental records for at least the periods required by our regulatory college and applicable law, and destroy them securely when they are no longer required.
Your Rights
- Access: you may request access to your dental records, subject to limited exceptions under PIPA and PIPEDA;
- Correction: you may request correction of information you believe is inaccurate or incomplete;
- Questions and complaints: you may raise any concern about our privacy practices with us directly.
Complaints
If you are not satisfied with our response to a privacy concern, you may contact the Office of the Information and Privacy Commissioner for British Columbia or the Office of the Privacy Commissioner of Canada. You will not be penalized for raising a concern.
